Terms and Conditions

I declare that I am the person with parental responsibility for the young person named in this form.

I give permission for my child/young person to participate in the children/youth programmes provided by Baptist Assembly.

I understand that the information used in this form will be used by the Children’s/Youth Team to help fulfil their roles in looking after this child/young person. As a Parent/Carer I will be expected to collect my child/young person from their respective groups promptly at the end of each session.

In the unlikely event of illness or accident, I give permission for any appropriate first aid to be administered to my child by the nominated first aider. In an emergency, and if I cannot be contacted, I am willing for my child to be given hospital treatment, including anaesthetic, if necessary. I understand that every effort will be made to contact me as soon as possible

I also understand that if my permission is given, photographs and/or video images may be taken at the event by team members or by other volunteers or members of staff of the Baptist Assembly.

I understand that these photographs and/or video images may be used internally or in promotional material. I understand that these images will not normally be of individual young people but will be trying to get a representation of the event and the activities.

I understand that due to the nature of public events the Baptist Assembly team cannot guarantee that other attendees will not take personal photos, particularly in the main auditorium.

I understand that at all times when my child/children are not in a specific children/youth programme session, I am responsible for supervising them and ensuring their safety for the duration of the Baptist Assembly.
