This form provides a tool to help ministers theologically reflect on their experiences in ministry. It will help ministers to identify Continuing Ministerial Development objectives, or to capture thoughts to be discussed with whoever accompanies them, such as a mentor, pastoral supervisor or spiritual director. It is taken from Appendix 6 of the CMD handbook pdf, but is presented here as a Word document on its own that may be filled in and expanded as required.
