This is a blank report form to be completed by NAMs' mentors in discussion with their NAM after the first few months of their ministry as a NAM, and again at tthe end of each year of the NAMs' programme. It is based on the Marks of Ministry but also looks for commentary on the NAMs' competence to minister. It is taken from NAMs' Programme pdf, but is presented here as a Word document on its own that may be filled as required.
